NEW CISD Enrollment

Account Request

This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
IMPORTANT: This enrollment application is for NEW to CISD students and former CISD students who are enrolling for the first time this year While completing the application, you will be asked to submit the following documentation. You may scan them in or take a photo of them with your phone, either will suffice. 1. Copy of the child's birth certificate. 2. Social Security Card 3. Withdrawal paperwork from the previous school attended, if available 4. Proof of Residence (A current water, electric or gas bill; no disconnect notices or telephone bills will be accepted). 5. Driver's License of person enrolling student. 6. Current Immunization Records. Please ensure you are choosing the correct year to enroll. Select CURRENT YEAR to enroll for the remainder of 2022-23.
